Lou's Quality Home Health Care Services is a State-licensed Home Healthcare Agency, a locally owned and managed company. LQHHCS staff and patients work together to achieve an optimal Care Plan tailored for each client. Our caregivers recognize the significance of clients desiring to remain in the familiar setting of their homes and strive to provide the best home care possible along with dedication and compassion.
LQHHCS Schedulers/Receptionists are accessible daily throughout the year. Schedulers are available during the day and after hours to assist you with your questions daily from 6 AM to 9 PM for your convenience. Including weekend and holiday access to our Schedulers improve communication considerably for our clients and caregivers.
Registered Nurses visit patients once a month either directly or by video conferencing. Additional assessments are scheduled if the client's condition necessitates further evaluation.
During visits, RN observes the patient's status, documents the condition, and notes changes from the most recent visit; if noticeable changes are apparent, the RN will discuss the patient's condition with a family and, if necessary, with the patient's Primary Care Provider.
Following the discussion with the PCP, the RN and Caregiver will follow the latest orders if new Verbal Orders are ordered, thus rendering optimum care for the patient.
After each visit, the RN enters the relevant information into LQHHCS's proprietary patient Database.
Caregiver's duties:
Nursing Aides assist with activities of daily living(ADL), bathing, dressing, grooming, toileting, light housekeeping, food preparation, feeding, walking assistance, and transferring bed to chair or wheelchair. Related Caregiver's duties include cleaning areas after cooking, bathing and maintaining cleanliness, and keeping the patient's living space clear of obstructions.
Certified Nurse's Assistant duties: Scope of work includes but is not limited to bathing, oral care, catheter care, and restorative care, i.e., ambulation and exercise (ROM).
Patient Care Technician (CNA II) duties: Scope of work, perform a broader range of medical procedures than a CNA. PCT performs enhanced medical procedures and routinely works with RNs to include, but not be limited to, dressing changes, tube feedings, Ambu Bag, blood sugar testing, and sample collection.
Nursing Aides and Certified Patient Care Technicians provide bedside care to patients and clients' homes, hospitals, or retirement community facilities.
New Client, before the caregiver's first visit, the caregiverʻs photo ID is sent via Email or Text, assuring the authenticity of their caregiver.
Minimizing Tardeness/No Show. Schedulers are tasked to remind caregivers via text of their schedule before their next care service.
